Advocate Good Shepherd Hospital
Diabetes Health Fair
When: Saturday, April 21st from 8 a.m. to noon
Location: Advocate Good Shepherd Hospital Outpatient Center—Crystal Lake
525 Congress Parkway, Crystal Lake
Cost: $10.00
Screenings will include a finger stick blood glucose level (12 hour fast is required—do take regular medications) and an A1C Level.
All patients will have the opportunity to meet with our Endocrinologist, Daniel So, M.D., and our Diabetes Educators.
Follow up screenings will be made if necessary.
