Description
Advocate Good Shepherd Hospital and Ela Township Community Center are partnering up to help provide diabetes health screenings. Screenings include a finger stick blood glucose level (12 hour fast is required) and an A1C Level. Patients have the opportunity to meet one-on-one with Daniel So, MD, Endocrinologist or a Good Shepherd Diabetes Educators. Complimentary foot screenings will also be provided. The Diabetes Health Screening is open to all community members.
